Appointment will be made on GRADE V – CLERICAL (€51,206 - €61,253 LSI) at a point in line with Government pay policy.

Position Summary:

 

Grade V Post Summary (General)

The post holder must have a good knowledge of the health service/ public service environment. The individual requires a working knowledge of the Nursing Home Support Scheme and the Local Health Offices procedures for Home Care Package applications as well as SOPs with regards to Complex Discharge Planning. The individual will also be required to support Patient Flow with regards to coordination of off-site beds. The individual would understand the challenges of delivering hospital services at the frontline and be able to make connect ions between the delayed discharge process and its impact on the wider hospital.

The role also involves;

  • Providing a variety of administrative functions
  • Assisting in planning, co-ordinating and directing daily operations of delayed discharge processes
  • Awareness of Departmental KPI’s, goals and objectives

Responsibilities

 

 

Operational management

  • Participate in the development and management of appropriate and relevant SOPs
  • Establish good office procedures to organise and manage departmental processes
  • Be familiar with the appropriate standards in relation to submission of external documentation
  • Ensure all paperwork relevant to specific discharge pathways is completed accurately
  • Work within internal and external deadlines, ensuring valid data and information is produced within a timely manner
  • Co-ordinating Transitional Funding for long/short term care and follow up with extensions when required

 

 

Data Quality and Reporting

  • To ensure that the data in respect of all activity within the Delayed Discharge database is of the highest quality
  • Be accountable for the validation of current and historical data gathered by the delayed discharge database
  • Provide accurate standard reports as well as specifically requested data/ information as and when required, in a timely manner

Communication

  • Ensure good communication amongst Patient Flow, the Social Work Department and Clinical Staff • Liaise directly with relevant HSE personnel, developing and maintaining collaborative and productive relationships
  • Act with professionalism and discretion with all communications with families and patients throughout this process
  • Provide support and liaise with families and staff with regards to the Long Term Care and Home Care Package processes and completion of essential documentation
  • Liaise with referring Geriatric Consultants to coordinate Local Placement Forum documentation

Staff Development/Training

  • Identify own training needs with regard to the use of technologies as they apply to role
  • Keep up to date with and developments in Home Care Packages and Nursing Home Support Scheme funding and application protocols and procedures
  • To support the continuous development and maintenance of an effective and efficient delayed discharge process
  • To support the design and delivery of education programmes to ensure the correct implementation of SOP’s

General Duties

  • Encourage good working relationships within the multidisciplinary and interdepartmental staff involved in the discharge processes
  • Identify opportunities for improvement and pursue same
  • Assist in identification of cost improvements, service developments and income generation opportunities within the Department
  • Act with discretion and integrity at all times in relation to sensitive data linked to the department or the hospital
  • Attend and minute meetings as required. Perform all other relevant administrative duties as required

Qualifications

 

 

Mandatory

 

1. Professional Qualifications, Experience, etc


(a) Eligible applicants will be those who on the closing date for the competition:


(i) Have satisfactory experience as a Clerical Officer in the HSE, TUSLA, other statutory
health agencies, or a body which provides services on behalf of the HSE under Section
38 of the Health Act 2004.


Or


(ii) Have obtained a pass (Grade D) in at least five subjects from the approved list of subjects
in the Department of Education Leaving Certificate Examination, including
Mathematics and English or Irish1. Candidates should have obtained at least Grade C
on higher level papers in three subjects in that examination.


Or


(iii) Have completed a relevant examination at a comparable standard in any equivalent
examination in another jurisdiction.


Or


(iv) Hold a comparable and relevant third level qualification of at least level 6 on the
National Qualifications Framework maintained by Qualifications and Quality Ireland,
(QQI).Note1:


Candidates must achieve a pass in Ordinary or Higher level papers. A pass in a
foundation level paper is not acceptable.


Candidates must have achieved these grades on the Leaving Certificate Established
programme or the Leaving Certificate Vocational programme.


The Leaving Certification Applied Programme does not fulfil the eligibility criteria.

(b) Candidates must possess the requisite knowledge and ability, including a high standard of
suitability, for the proper discharge of the office.


2. Age


Age restriction shall only apply to a candidate where s/he is not classified as a new entrant (within
the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act, 2004). A
candidate who is not classified as a new entrant must be under 65 years of age on the first day of
the month in which the latest date for receiving completed application forms for the office occurs.


3. Health


Candidates for and any person holding the office must be fully competent and capable of
undertaking the duties attached to the office and be in a state of health such as would indicate a
reasonable prospect of ability to render regular and efficient service.


4. Character


Candidates for and any person holding the office must be of good character.

 

 

 

Desirable:

 

  • Knowledge of The Nursing Home Support Act 2009 ·
  • Experience in coordination of complex discharge processes ·
  • Critical analysis skills (including data, reporting and presentation)
  • Good knowledge of electronic systems i.e. BHIS and Diver
  • High level of IT literacy – word, excel, power point
  • Ability to collate, analyse and interpret data
  • Experience of managing a change in process/ systems
